About the Role

An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or alternate infusion suite, ensuring patient safety with continuity and compliance under a physician’s plan of care. Works under structured supervision of the designated supervisor.

Responsibilities

  • Initiates, develops, and implements an intermediate nursing plan of care, evaluating patient progress toward goals.
  • Organizes and participates in direct patient care, including administering treatments, medications, and educating patients and families/caregivers.
  • Modifies the plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals.
  • Assesses patient needs and physical status at each skilled visit through health data access and patient interviews; re-evaluates needs through reassessment and response to therapy.
  • Obtains and clarifies physician orders for plan of treatment revisions; informs physicians promptly of significant changes in patient condition and provides written summaries.
  • Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
  • Delivers effective and safe teaching using a patient-centered approach to help patients and families achieve independence with prescribed therapy.
  • Communicates effectively and timely with the clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care.
  • Prepares clinical documentation in real-time during visits and submits it to the nursing department to comply with billing timelines.
  • Coordinates discharge planning, prepares discharge summaries, and thoroughly reports patient care needs when transferring care.
  • Demonstrates compliance with agency operations, policies, procedures, and professional standards, including local, state, and federal regulations.
  • Maximizes efficiency through the use of computers and automation technologies for validating orders and communicating care.
  • Responds promptly to patient requests and initiates emergency procedures as necessary.
  • Accepts accountability for ethical and professional conduct, adhering to Option Care’s mission, values, and objectives.
  • Safeguards patient confidentiality and proprietary information, including adherence to HIPAA regulations.
  • Educates patients about Option Care’s scope of services and related financial obligations.
  • Completes required training, in-services, and continuing education to maintain competency and licensure.
  • Provides oversight and coordination of paraprofessionals in the home setting; may act as Supervisor in the absence of the designated supervisor.
  • Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses.
  • Participates in nursing department on-call responsibilities.
  • Performs other related duties as directed by supervisor.
  • If applicable: Inserts PICC/Midlines per physician order, troubleshoots/declots obstructed central line catheters, and monitors/educates on prevention of line infection.

Requirements

  • Active and unrestricted RN license in the state of practice.
  • Minimum of 1 year of nursing experience.
  • Current CPR certification.
  • Demonstrated competency in patient care standards for safe delivery of services and infusion skill sets.
  • Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel and proof of liability insurance.
  • Basic knowledge of computer operating systems and software applications for effective use of nursing technology tools.

Preferred Qualifications

  • Bachelor of Science in Nursing from an accredited school of nursing.
  • 1-2 years of previous infusion nursing experience.

Physical Demands

  • Ability to lift up to 50 pounds, with a maximum lifting requirement of 75 pounds.
  • Exerting up to 20 pounds of force occasionally or up to 10 pounds frequently.
  • Physical demands may include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying.
  • Fine motor skills and visual acuity, including close and distance vision, color and peripheral vision, depth perception, and ability to adjust focus.
  • Likelihood of occupational exposure to blood, body fluids, and other potentially infectious materials.
  • Possible exposure to hazardous substances, adverse weather, animals, secondhand smoke/vape, and unpredictable home environments.
  • Safety requirements include closed-toe shoes, facemask, goggles, gown, gloves, and a functioning communication device.

Travel Requirements

Willing to travel up to 100% of the time for business purposes.

Pay

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Pay range is $40.78–$67.97 per hour.
