About The Role
We are seeking a physician who provides reasonable and prudent medical care, exercises sound clinical judgment grounded in best-practice standards and evidence-based medicine, and delivers care that aligns with their professional licensing, training, and credentialing.
Key Responsibilities
- Counsels patients on health maintenance, disease prevention, and health promotion.
- Maintains patient confidentiality in compliance with agency, state, and HIPAA laws.
- Affirms medical and psycho-social histories, performs physical exams, identifies problems, formulates treatment plans, provides education and referrals, and evaluates intervention effectiveness.
- Orders and interprets laboratory tests, reports communicable diseases to the Health Department as per protocol.
- Prescribes appropriate medication.
- Implements standardized evaluations or assessments with a multidisciplinary team.
- Ensures complete, timely, and accurate documentation.
- Understands medical emergency procedures and the location of emergency supplies.
- Participates in continuing medical education to enhance professionalism and quality of care.
- Provides on-call coverage as scheduled.
- Maintains good working relationships with community agencies.
- Attends and participates in team conferences, huddles, and required trainings.
Required Qualifications
- Education: Degree (Nurse Practitioner or Physician Associate) from an accredited Nursing or Physician Associate school.
- CPR Certification required.
- Experience: Board Certification or Board Eligibility. (Board certification must be attained within the timeframe dictated by one’s specialty in order to remain on staff at the CS-HHC if one was originally Board Eligible at the time of one’s hire.) Connecticut Licensure. Must have at least 5 years of clinical experience in outpatient internal medicine or family practice. Prior experience working in addiction medicine preferred.
