Infant and Child Case Manager (2112)

Responsibilities

• Provides assessment, linkage, and advocacy services to individuals from birth to seven who are at risk of serious emotional disturbance.
• Acts as the primary facilitator in coordinating and integrating services for the assigned caseload.
• Develops and implements Individual Service Plans (ISPs).
• Completes required documentation.
• Complies with all applicable regulations governing program services.
• Applies generally prescribed standard practices over a wide range of procedures with some latitude for decision making.

Requirements

• Minimum Requirements:
  • M1: Bachelors Degree in Psychology, Social Work, Criminal Justice, Vocational Rehabilitation, Counseling, Sociology, or other approved Human Services degree
  • M2: Registered Nurse with a valid Registered Nurse license to practice in Virginia
• Valid drivers license, safe driving record, and reliable transportation
• Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request.
• The Agency maintains a drug-free workplace.

Pay

• Annuial Salary Range: $43,722 - $76,515
• 5% Increase for Qualified Mental Health Professional
