About the Role
The Individual Supports Case Manager is responsible for documentation and maintenance of assigned Individual Supports placements, assessments, records, plan of care, and suitability of living arrangement. The Case Manager must proactively focus on the individual in supports’ advocacy, empowerment, and community integration. There is also a strong focus on resource development and delivery to retain placements. This role involves travel around the North and South Central areas of Massachusetts. Some office time in Sturbridge and/or Uxbridge may be required.
Responsibilities
- Participate in group supervision meetings as scheduled
- Provide recommendations to your supervisor regarding ways to improve the current system
- Observe all Agency policies and procedures
- Accept responsibility and accountability as part of the position
- Maintain confidentiality and positive demeanor while successfully managing multiple priorities simultaneously
- Maintain current CPR, AED, and First Aid Certifications
Requirements
- Ability to establish rapport with the disabled population, their families, co-workers, and service providers
- Ability to work independently and as part of a team
- Ability to plan and prioritize work with limited supervision
- Ability to travel 10 to 60 miles on a daily basis
- Organized, dependable, and even temperament; good phone manner
- Proficient in computer usage
- Effective verbal and written communication skills
- Strong interpersonal and communication skills, and the ability to work with a wide range of individuals
- Must be able to bend, lift, stoop, reach, and file
Qualifications
- B.A. or B.S. Degree preferred and/or at least two years’ experience in working as a case manager with elderly or disabled adults
- Knowledge of the Department of Developmental Services regulations a plus
- CPR, AED, and First Aid Certifications
- Reliable transportation and flexible hours is a must
- Must be 18+ years old, have a valid driver's license, and be available by telephone for daily contact
Pay
This position will be compensated on an hourly basis.
