Individual Supports Case Manager

Requirements

• Participate in group supervision meetings as scheduled
• Provide recommendations to your supervisor regarding ways to improve the current system
• Observe all Agency policies and procedures
• Maintain confidentiality and positive demeanor while successfully managing multiple priorities simultaneously
• Maintain current CPR, AED and First Aid Certifications

Physical And Mental Requirements

• Establish rapport with the disabled population, their families, co-workers and service providers
• Work independently and as part of a team
• Plan and prioritize work with limited supervision
• Bend, lift, stoop, reach and file

Minimum Acceptable Qualifications

• B.A. or B.S. Degree preferred and/or at least two years’ experience in working as a case manager with elderly or disabled adults
• Knowledge of the Department of Developmental Services regulations a plus

Physical Demands

• Reliable transportation and flexible hours is a must
• Must be 18+ years old, a valid driver's license and be available by telephone for daily contact
