Responsibilities

  • Provide comprehensive case management services to identified chronic homeless individuals.
  • Manage all aspects of assigned case load, including meeting with clients regularly both in the field and in the office.
  • Ensure all clinical files and reports are complete and up to date.
  • Conduct comprehensive assessments to identify barriers to housing and assist clients in overcoming these barriers to secure entitlements.
  • Develop and work with clients on individualized housing plans, building a network of support services.
  • Prepare clients for housing interviews and arrange housing appointments.
  • Participate in case conferences as necessary.
  • Liaise with clinical staff in other BRC or outside programs regarding common clients.
  • Provide emergency first aid/CPR or overdose prevention protocol assistance when needed.
  • Perform related duties as assigned.

Qualifications

  • High School Diploma/GED required; bachelor's degree preferred and master's ideal.
  • Bilingual (Spanish) strongly preferred, but not required.
  • Excellent communication and writing skills, and computer proficiency required.
  • Experience working in residential treatment and with the MICA and homeless populations.
  • Knowledge of HRA and NY/NY application systems a plus.
  • CPR training certification or willingness to take a CPR training class.
  • Vaccination preferred but not required.

Schedule

Full-time, 37.5 hours per week.
