Inclusion Specialist

About the role

As an Inclusion Services Specialist at New Avenues to Independence, you will support and empower individuals with disabilities, helping them lead fulfilling lives and become active members of their community.

Responsibilities

• Accompany individuals to job sites; observe performance; oversee work training; provide job retention training, instruction and assistance.
• Provide support to individuals and team members in obtaining participant information to assess employment history, education, skills, abilities and potential barriers to employment.
• Function as an advocate and customer service liaison between site personnel and individuals to maintain open communication and achieve community inclusion goals.
• Ensure adherence to HIPAA standards at all times.
• Provide person-centered care and support to individuals with disabilities in residential homes and day programs.
• Assist individuals with daily living activities, including meal preparation, personal hygiene, and medication management.
• Foster a nurturing and inclusive environment that promotes growth, independence, and self-determination.
• Plan and engage in meaningful activities, outings, and events to enhance quality of life.
• Promote social and emotional well-being by building strong, supportive relationships.
• Collaborate with a team to create and implement individualized care plans.
• Collaborate with people to complete tasks and build independence.
• Provide support with daily routines such as personal care, cooking, cleaning, shopping and attending medical appointments.

Requirements

• 18 years of age or older.
• Valid Ohio Driver’s license with 4 points or less and valid auto insurance.
• High school diploma or GED.
• Desire to make a difference in people’s lives.

Pay

$17 per hour.
