Responsibilities
- Conduct in-home health evaluations and connect with patients personally
- Bridge gaps in care by providing tailored health assessments
- Review medication and medical history during evaluations
- Utilize digital tools for patient assessments and documentation
- Engage in local travel for home visits within licensed states
- Perform virtual visits as required
- Deliver diagnostic and preventive services based on individual health needs
Qualifications
- Active, unrestricted license(s) in coverage area(s) for MDs and DOs
- PREFERRED: Multi-state licenses
- Experience in conducting in-home health evaluations
- Strong interpersonal skills with a focus on patient engagement
- Ability to adapt to diverse home environments and conditions
- Familiarity with conducting comprehensive health assessments
