In-Home Nurse Practitioner or Physician Assistant Per Diem - Grand Rapids

About the Role

Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You will engage with individuals in their homes, helping bridge gaps in care by providing flexible, comprehensive, and personalized health evaluations.

The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.

Responsibilities

• Conduct in-home health evaluations, building trusted relationships to improve health outcomes.
• Perform medication and medical history reviews, physical evaluations, and diagnostic tests as needed (e.g., diabetic eye exams, spirometry, bone density screening).
• Travel locally and within licensed states to visit health plan members in their homes.
• Conduct virtual visits as required.
• Provide all Signify Health services, including Diagnostic & Preventive Services products.

Work Environment

As an in-home clinician, your primary workplace will be the private residences of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.

Requirements

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Pay

The typical pay range for this role is $41.85 - $90.13 per hour. The actual base hourly rate will depend on factors such as experience, education, and geography.
