In-Home Nurse Practitioner or Physician Assistant Per Diem - Lansing

About the Role

Join Signify Health, part of CVS Health®, as a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You’ll engage with individuals in the comfort of their homes, helping bridge gaps in care and building trusted relationships to improve health outcomes. The role involves conducting comprehensive health evaluations, including medication and medical history reviews, physical evaluations, and additional diagnostic tests if ordered by the health plan.

Each visit lasts about 45 minutes, allowing time to connect one-on-one and address health questions. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter treatment regimens.

Responsibilities

• Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., diabetic eye exams, spirometry, bone density screening).
• Build trusted relationships with health plan members to improve health outcomes.
• Travel locally and within licensed states to perform evaluations.
• Conduct virtual visits as needed.
• Provide all Signify Health services, including Diagnostic & Preventive Services products.
• Work in diverse home environments, which may include pets, secondhand smoke, or other airborne allergens.

Requirements

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Pay

The typical pay range for this role is $41.85 - $90.13 per hour. The actual base hourly rate will depend on factors such as experience, education, and geography.
