In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
As a Part Time Clinician (Nurse Practitioner or Physician Assistant) with Signify Health, part of CVS Health®, you’ll provide In-Home Health Evaluations, engaging with people in the comfort of their own homes to help bridge gaps in care. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
Your primary workplace will be the private residences of health plan members. These environments are diverse and may present varying conditions, including household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in these settings.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
- Perform additional diagnostic tests as ordered by health plans (e.g., diabetic eye exams, spirometry, bone density screenings).
- Build trusted relationships with members to improve health outcomes and provide personalized care.
- Travel locally and within licensed states as needed to conduct in-home visits.
- Conduct virtual visits when necessary.
- Provide all Signify Health services, including Diagnostic & Preventive Services products.
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).

Pay
The typical pay range for this role is $41.85 - $90.13 per hour. The actual base hourly rate will depend on factors such as experience, education, geography, and other relevant considerations.
Schedule
- Part-time: 29.5 hours per week.
