In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
Responsibilities
- Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person’s health plan
- Use an iPad and an intuitive clinical workflow for each evaluation; no prescribing, ordering lab tests, or altering treatment regimens required
- Build trusted relationships with health plan members to improve health outcomes
- Work with a national network of purpose-driven clinicians to visit millions of people nationwide
- Travel locally and within licensed states as needed; conduct virtual visits when required
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE)
Work Environment
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may include household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to safely perform duties in these conditions.

Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Able to cover the following primary locations: Houghton (Counties: Gogebic, Houghton, Keweenaw, Ontonagon)
Schedule
Part-time, 29.5 anticipated weekly hours
Pay
The typical pay range for this role is: $41.85 - $90.13 per hour. The actual base salary offer will depend on experience, education, geography, and other relevant factors.
