In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You’ll engage with people in the comfort of their own homes, helping bridge gaps in care and building trusted relationships to make people healthier. This role brings flexible, comprehensive and personalized health evaluations right to people’s front doors.
Responsibilities
- Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical evaluation, and additional diagnostic tests when ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Provide connections to the right care for people’s unique needs as part of a national network of purpose-driven clinicians.
- Travel locally and within licensed states as needed; conduct virtual visits when required.
- Deliver all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams, bone density screening).
Work Environment
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform duties in these settings.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).

Pay
The typical pay range for this role is $46.03 - $99.14 per hour. The actual base salary offer will depend on experience, education, geography and other relevant factors.
Schedule
Part-time, with anticipated weekly hours of 29.5.
