In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
As a Part Time Clinician (Nurse Practitioner or Physician Assistant) with Signify Health, part of CVS Health, you’ll conduct in-home health evaluations to engage with individuals in their own homes, bridging gaps in care and building trusted relationships. Each visit lasts approximately 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the member’s health plan. You’ll use an iPad with an intuitive clinical workflow and are not responsible for prescribing medicine, ordering lab tests, or altering treatment regimens.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
- Perform diagnostic and preventive services (e.g., spirometry, diabetic eye exams, bone density screenings) as indicated.
- Travel locally and within licensed states to visit health plan members in their homes.
- Conduct virtual visits as needed.
- Provide education and guidance to help individuals better understand and manage their health.
- Collaborate with a national network of clinicians focused on improving health outcomes.
Work Environment
Your primary workplace will be the private residences of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).

Pay
The typical pay range for this role is $41.85 - $90.13 per hour, depending on experience, education, geography, and other relevant factors.
