About the Role

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You’ll engage with individuals in the comfort of their homes, helping bridge gaps in care and building trusted relationships to improve health outcomes. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the person’s health plan. You will use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter treatment regimens.

Responsibilities

  • Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
  • Perform diagnostic and preventive services as indicated (e.g., spirometry, diabetic eye exams, bone density screening).
  • Travel locally and within licensed states to visit health plan members in their homes.
  • Conduct virtual visits as needed.
  • Build trusted relationships with members to address their health questions and concerns.
  • Provide health education and guidance to help individuals better understand and manage their health.

Work Environment

As an in-home clinician, your primary workplace will be the private residences of health plan members. These environments vary and may include conditions such as household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.

Qualifications

  • Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the coverage area(s).
  • Board certification (required).
  • Multi-state licenses (preferred).
  • Able to cover the specified primary locations.

Pay

The typical pay range for this role is $87,035.00 - $187,460.00 annually, depending on experience, education, geography, and other relevant factors.

Schedule

  • Full-time position with anticipated weekly hours of 40.
