About the role

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, primarily serving pediatric patients.

Responsibilities

• Deliver and document nursing care according to physician orders and care plans.
• Assess patient conditions, report changes, and implement appropriate interventions to ensure safety and health outcomes.
• Educate patients and families on disease management, self‑care, and prevention strategies.
• Maintain accurate records of assessments, actions, and responses in compliance with regulations.
• Adhere to infection control protocols and professional standards.

Qualifications & Skills

• Active, unrestricted LPN/LVN license in the practicing state.
• Current CPR certification and a recent TB test (within 12 months).
• Six months of clinical nursing experience preferred; home health experience a plus.
• Strong attention to detail, organization, problem‑solving, and communication skills.
• Reliable transportation and physical ability to lift up to 50 pounds and perform patient repositioning.
• Ability to function effectively in diverse environments and respond to emergencies.
