Responsibilities

• Provide compassionate nursing care to adult patients in a home setting, managing acuity cases such as feeding tubes, tracheostomies, and medication administration (training available).
• Ensure clinical outcomes through personalized patient care and documentation using electronic systems.
• Support patients and families, and collaborate with the healthcare team to deliver quality homecare.

Qualifications

• Maintain an active, unencumbered LPN/LVN license in the practicing state.
• Current CPR certification.
• TB test within the last 12 months.
• Reliable transportation.
• Preferred six months of hands-on nursing experience.
• Transfer of compact licenses to the state of residence must be completed within 90 days if applicable.
