About the role
Licensed Practice Nurse (LPN) providing home healthcare services to patients in a supportive environment.
Responsibilities
- Assess, plan, and implement personalized patient care
- Educate patients and families on disease management, procedures, and medications
- Manage wound care, tracheostomy, g-tubes, catheters, ventilators, and medication administration
- Develop customized care plans to meet individual needs and preferences
Qualifications & Skills
- Valid LPN license
- Excellent clinical skills
- Ability to work independently
- Strong communication and patient education skills
- Adaptability to diverse case types including pediatric and adult cases
Key Duties
- Provide comprehensive in-home care
- Build long-term patient relationships
- Support patient health milestones
- Collaborate with a clinical team
