In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Bronx, NY
About the role
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations.
Responsibilities
- Engage with people in the comfort of their own homes to provide health evaluations.
- Conduct a medication and medical history review, physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry, etc.).
- Use an iPad and an intuitive clinical workflow for each evaluation.
- Do not prescribe medicine, order lab tests, or alter people’s current treatment regimen.
- Build trusted relationships to make people healthier and help bridge gaps in care.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening, etc.).
Qualifications
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the following primary locations: Bronx and surrounding area.

Pay and Schedule
The typical pay range for this role is $95,738.00 - $206,206.00. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant criteria. This position is eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventive Services (DPS), as ordered, in addition to the base pay listed above. The anticipated weekly hours for this position are 40.
