In Clinic Visit Facilitator- Pediatric Clinic

Fairview has an immediate opening for an In Clinic Visit Facilitator to join our Pediatric Care team at our Riverside Clinic in Minneapolis, MN. This is a 0.8 FTE (64 hours per two-week pay period), benefits-eligible position.

Schedule

• Monday–Thursday: 7:00 AM–6:00 PM
• Friday: 7:00 AM–5:00 PM
• No weekends

Responsibilities

• Prepare patients for appointments by communicating in advance to ensure medical records are collected, received, and available for the visit.
• Assist in the rooming process during in-person visits by collecting and documenting pertinent information such as reason for visit, patient-reported medication review, vitals (using an automated vitals machine), and applicable health questionnaires.
• Prepare exam rooms and stock supplies.
• Support patient-provider connection during visits, ensuring a positive experience for both.
• Complete visit wrap-up, including providing after-visit summaries and assisting with scheduling or coordinating follow-up appointments.
• Communicate effectively with patients, providers, and other health professionals, both in-person and using virtual technologies.
• Assist with scheduling, coordinating appointments, and other delegated tasks to support team efficiency.

Requirements

• 1 year of customer service experience.
• Proficient with technology (computer hardware, programs, smart devices such as phones or tablets).

Preferred Qualifications

• Current student in nursing, medicine, social work, psychology, or pharmacy.
• Successful completion of a Nursing Assistant, Emergency Medical Technician, or Health Unit Coordinator program.
• Knowledge of medical terminology.
• Experience with Epic Health Records.
• Telephonic customer service experience.
• Previous experience working in a healthcare setting.

Pay

Compensation is determined by factors such as skills, knowledge, relevant education, experience, and market conditions. Pay equity and internal team equity are prioritized. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of hire.
