Fairview has an immediate opening for an In Clinic Visit Facilitator at our Clinics and Surgery Center in Minneapolis, MN. This is a 1.0 FTE (80 hours per two-week pay period), benefits-eligible position. Clinic hours of operation are Monday - Thursday: 7:00AM - 6:00PM and Friday: 7:00AM - 5:00PM.

Patients travel from across the country and around the world for treatments at our facilities. The team engages in high-impact work in cancer prevention, detection, and treatment.

Visit Facilitators are a valuable member of our care team, responsible for supporting in-person clinic visits, including communicating with patients prior to their visit and guiding them through the care experience.

Responsibilities

  • Prepare patients for appointments by communicating with them in advance to ensure medical records are collected, received, and available for the visit, if applicable.
  • Assist in the rooming process during in-person visits by collecting and documenting pertinent information such as the reason for the visit, patient-reported medication review, vitals collection using an automated vitals machine, and applicable health questionnaires.
  • Prepare exam rooms and stock supplies.
  • Support the patient connection to the provider during the visit and serve as a resource to ensure a positive patient/provider experience.
  • Complete visit wrap-up by providing the after-visit summary and ensuring patients are aware of follow-up recommendations, which may include assisting with scheduling and coordinating follow-up appointments.
  • Communicate effectively with patients, providers, and other health professionals, demonstrating comfort with in-person and virtual technologies.

Requirements

  • At least 1 year of customer service experience.
  • Proficient with technology (computer hardware and programs, smart devices such as phones or tablets).

Preferred Qualifications

  • Current nursing, medical, social work, psychology, or pharmacy student.
  • Successful completion of: NA, EMT, or HUC program.
  • Experience with medical terminology, Epic Health Records, telephonic customer service, or previous work in a healthcare setting.

Pay

An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, the organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of the offer.
