Position Overview
The ID/DD Coordinator is responsible and accountable for the coordination and administration of services for clients and their caregivers under the direct supervision of the Location or Executive Director.
Essential Job Functions
- Provide initial evaluation to determine appropriateness and safety of client and environment.
- Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws.
- Write a summary for individual clients and services based on needs, goals, and expected outcomes in conjunction with the ISP.
- Supports Coordinators/Administrative Entities on a monthly basis.
- Participate in the development of Individual Support Plan.
- Maintain patient confidentiality.
- Demonstrate an understanding of client's medical status and updates self on clients/family status as needed or requested.
- Maintain effective communication between staff, supports coordinators, and family.
- Review progress notes from paraprofessionals to determine appropriate service delivery.
- Education paraprofessionals on individual client ISP expectations.
- Perform on-site evaluation of employee performance at 90 days and annually.
Requirements
- Bachelor’s degree in Psychology or other social science or related field.
- Current CPR certification.
- Proof of eligibility to work in the United States.
- Current completion of child abuse clearance and Fingerprinting.
Preferences
- Minimum of one year of experience with ID/DD preferred.

Physical Requirements
- Must be able to speak, write, read and understand English.
- Must be able to travel; company does not provide vehicles or transportation.
- Frequent lifting, carrying, pushing and pulling of 25 pounds.
- Occasional lifting, carrying, pushing and pulling of 50 pounds.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have strong sense of smell and touch.
Environment
- Must be able to function in a wide variety of environments which may involve exposure to allergens and other various conditions.
- May involve possible exposure to blood, bodily fluids and infectious diseases.
