About the role
The ICMS Case Manager will provide coordinated Case Management and Supportive Services for formerly homeless individuals diagnosed with special needs. Services will be delivered on-site to tenants residing at a newly developed multi-unit permanent supportive housing complex.
Responsibilities
- Work with each tenant to conduct an initial housing intake and needs assessment
- Aid tenants in setting personal goals and developing Individual Service Plans (ISPs)
- Support clients in managing their health conditions and accessing necessary services
- Conduct at least two face-to-face contacts with each tenant monthly
- Meet with tenants quarterly to review progress and update Individualized Stability Plans
- Act as a liaison between the property manager, property management company, tenant, and primary mental health provider
- Provide information and instruction to clients about Fair Housing, tenant rights and responsibilities, and other relevant topics
- Coordinate with various public social service agencies and community providers to assist clients in achieving independence
- Assist clients in securing rental assistance and resolving tenant grievances
- Enforce housing rules in collaboration with property management
- Develop and lead workshops on relevant topics to support tenants' independence
- Collect and enter data into DHS CHAMP’s database to document client progress and report outcomes
- Manage client files and monitor tenants transitioning to mainstream housing
- Comply with service contract requirements and submit monthly progress reports
Requirements
- 1-2 years of experience or related work
- Bachelor’s degree
- Experience working with youth experiencing homelessness and housing instability, and/or experience in foster care, probation, mental health systems
- Reliable, flexible team player with a good work ethic and ability to set personal boundaries
- Valid driver's license and car insurance as required by law
- Proficiency in Microsoft Office suite
