ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Dispensing & Operational Responsibilities:
    • Interprets written prescriptions; evaluates for appropriateness including drug interactions, allergies, and therapeutic duplications.
    • Enters prescription and patient data into pharmacy systems and supervises Pharmacy Technicians in preparation and dispensing.
    • Verifies accuracy of all prescriptions prior to dispensing.
    • Maintains inventory, including ordering medications and ensuring appropriate stock levels without overstocking.
    • Maintains perpetual Schedule II inventory and ensures controlled substance compliance.
    • Resolves third-party rejections and maintains required daily and weekly reports.
    • Follows company procedures for dispensing incidents, including documentation and notification.
    • Maintains working knowledge of pharmacy systems and operations.
  • Clinical & Patient Care Responsibilities:
    • Provides medication counseling to patients, residents, and caregivers to ensure understanding of therapy, goals, and proper medication use.
    • Communicates with prescribers and other healthcare professionals to optimize patient outcomes.
    • Conducts Medication Regimen Reviews (MRRs) as required by regulation; identifies, documents, and reports medication-related issues and recommendations to facility staff.
    • Evaluates medication therapy for appropriateness, effectiveness, safety, and adherence.
    • Provides disease state education and supports achievement of therapeutic goals.
  • Consultant & Facility Support Responsibilities:
    • Participates as a member of the interdisciplinary care team, contributing to resident assessments and care planning.
    • Attends and contributes to facility committees including: Quality Assurance and Performance Improvement (QAPI), Pharmacy Services, Infection Control, and Care Planning.
    • Affords assistance to facilities in developing, implementing, and maintaining policies and procedures related to: Medication ordering, storage, handling, labeling, administration, and destruction.
    • Reviews and monitors controlled substance utilization, reconciliation, and documentation.
    • Oversees and provides guidance on all aspects of the medication use process including prescribing, dispensing, delivery, administration, and disposal.
    • Provides staff education and in-services related to medications and regulatory requirements.
    • Conducts exit conferences with facility leadership (Administrator/Director of Nursing) as required.
    • Assists facilities with survey preparedness and provides support during regulatory surveys.
  • Compliance & Quality Responsibilities:
    • Ensures adherence to all applicable federal and state regulations governing long-term care pharmacy services.
    • Identifies trends in medication-related issues and participates in quality improvement initiatives.
    • Supports company and facility goals related to patient safety, compliance, and quality outcomes.
    • Maintains required licensure and continuing education.
  • General Responsibilities:
    • Maintains regular and punctual attendance.
    • Develops professional relationships with patients, facility staff, and colleagues.
    • Complies with all security and loss prevention procedures.
    • Supports overall company objectives and performs other duties as assigned.
    • Travels to partner facilities as needed.

EDUCATION and/or EXPERIENCE

  • Bachelor’s Degree in Pharmacy or PharmD from an accredited college of pharmacy.
  • Active and unrestricted Pharmacist license (state-specific as required).
  • 1–3 years of experience in long-term care, consultant pharmacy, or related setting preferred.
  • Strong clinical knowledge and regulatory understanding of long-term care pharmacy services.
  • Excellent interpersonal, communication, and organizational skills.

PHYSICAL REQUIREMENTS

  • Ability to sit, stand, walk, bend, and reach throughout the workday.
  • Ability to lift up to 50 lbs as needed.
  • Ability to communicate effectively with patients, healthcare providers, and team members.
