HVI Care Coordinator- Device Clinic Nurse

About the role

The HVI Care Coordinator manages comprehensive and individualized care for assigned patients and their families in seeking care at the Heart & Vascular Institute. This role assists the team with daily rounds, consultations, and evaluations. The coordinator reviews and communicates diagnostic test results, consults with clinic and hospital staff, and develops a comprehensive plan of care. They also participate in patient discharge planning and coordinate appropriate services.

Responsibilities

• Manages comprehensive and individualized care for assigned patients and their families in seeking care at the Heart & Vascular Institute.
• Affords assistance to the team during daily rounds, consultations, and evaluations.
• Reviews and communicates diagnostic test results, consults with clinic and hospital staff, and develops a comprehensive plan of care.
• Participates in patient discharge planning and coordinates appropriate services.
• Serves as a resource for clinical staff caring for assigned patients.
• Collaborates with outreach clinic locations to coordinate local follow-up when appropriate.

Requirements

• Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
• Obtain certification in Basic Life Support within 30 days of hire.
• Two (2) years clinical experience in nursing.

Preferred Qualifications

• Education, certification, and/or licensure in the area of specialty.
• Nursing experience in the area of specialty.

Core Duties and Responsibilities

• Manages comprehensive and individualized care for assigned patients and their families in seeking care at the Heart & Vascular Institute.
• Affords assistance to the team during daily rounds, consultations, and evaluations.
• Reviews and communicates diagnostic test results, consults with clinic and hospital staff, and develops a comprehensive plan of care.
• Participates in patient discharge planning and coordinates appropriate services.
• Serves as a resource for clinical staff caring for assigned patients.
• Collaborates with outreach clinic locations to coordinate local follow-up when appropriate.
• Serves as a professional role model.
• Participates and provides leadership for committees and task forces.
• Documents hours and submits billing reports, as appropriate.
• Reviews data from sources such as patient registries as necessary.
• Participates in clinical trials and research studies with the Principal Investigator (as applicable).

Physical Requirements

• Able to manually lift patients, and limit safe patient handling to 35 pounds.
• Work requires strength and/or stamina, lifting, moving, stooping, reaching, standing, walking, and carrying of materials and equipment weighing 40+lbs.

Working Environment
