About the role

The Care Coach evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity. The Care Coach will work directly with members enrolled in the Humana Beginnings program and may require in person visits with members. Care Coaches will employ a variety of strategies and techniques to support a member's optimal wellness state by coordinating services & resources.

Responsibilities

  • Identify and resolve barriers that hinder effective care.
  • Ensure patient is progressing towards desired outcomes by monitoring patient care through use of assessment, data, conversations with member, and active care planning for prenatal and postnatal members.
  • Work directly with members enrolled in the Humana Beginnings program and may require in person visits with members.
  • Employ a variety of strategies and techniques to support a member's optimal wellness state by coordinating services & resources.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Requirements

  • South Carolina Resident
  • LPN, RN or bachelor's degree in health and human services field
  • 1 or more years of maternity experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal/written communication and interpersonal skills
  • Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work

Preferred Qualifications

  • Mother Baby experience
  • Prior experience with Medicare & Medicaid recipients
  • Previous Care management experience
  • Previous experience with electronic case note documentation and with documenting in multiple computer applications/systems
  • Knowledge of community health and social service agencies and additional community resources

Additional Information

  • Work-At-Home Requirements: To Ensure Home Or Hybrid Home/Office Associates' Ability To Work Effectively, The Self-provided Internet Service Of Home Or Hybrid Home/Office Associates Must Meet The Following Criteria At Minimum, A Download Speed Of 25 Mbps And An Upload Speed Of 10 Mbps Is Recommended; Wireless, Wired Cable Or DSL Connection Is Suggested.
  • If Selected For This Role, You Will Be Required To Be Screened For TB.
  • Driving Statement: This Role Is Part Of Our Company's Driver Safety Program And Therefore Requires An Individual To Have A Valid State Driver's License And Are Expected To Maintain Personal Vehicle Liability Insurance.
  • Medicaid Mileage Reimbursement: We Provide Mileage Reimbursement For Work-Related Travel. Eligible Mileage Includes: Travel From Your Home To Your First Work Location Of The Day, Travel Between Client Or Assignment Locations During The Workday, Travel From Your Final Work Location Back To Your Home.
  • Travel: While This Is A Remote Position, Occasional Travel To Humana's Offices For Training Or Meetings May Be Required.
  • Scheduled Weekly Hours: 40
  • Pay Range: $53,700 - $72,600 per year
