About the role

The Human Services Counselor I position involves providing case management, care coordination, and parenting support to pregnant women and families of infants. This role requires regular home visits for up to 5 years and collaboration with a multi-disciplinary team.

Responsibilities

• Initiates and maintains regular client contact as per Healthy Families America accreditation standards.
• Visits families in their homes for up to 5 years to provide case management, care coordination, and parenting support.
• Serves as the primary coordinator on a multi-disciplinary team including therapists, nurses, and father support specialists.
• Maintains a case weight according to program requirements and ensures compliance with Healthy Families Pinellas guidelines.
• Links clients to community supports and services and tracks prenatal and infant health care.
• Communicates and coordinates with healthcare providers and ensures continued participation in health care.
• Documents case management, client progress, and home visiting activities using Healthy Families Pinellas forms.
• Maintains an electronic health record for each client and completes service ledgers for HFF data tracking.
• Completes encounters and EARS in HMS to document direct services, risk factors, service checks, and other work activities.
• Completes all HVRs within program standards.
• Maintains standards and applicable regulations for personnel, medical records management, and programmatic/funder requirements.
• Maintains a flexible schedule to meet the needs of families, including evenings and weekends.
• Meets grant goals, performance measures, and objectives, as well as programmatic and funder requirements.
• Meets weekly with supervisor for reflective, clinical, and administrative supervision.
• Levels cases according to need and participates in regular staff meetings.
• Attends training to promote professional and personal growth and completes all mandatory trainings as required.

Requirements

• At least one year of experience working with at-risk children and/or families.
• Valid driver's license with daily access to a reliable personal vehicle.
• Ability to work a flexible schedule, including evenings and weekends.
• Knowledge of basic computer software and ability to enter data in a computer.

Qualifications

• Minimum qualifications: One year of experience working with at-risk children and/or families.
• Preferred qualifications: Bilingual with written and spoken fluency in English and Spanish, Infant Mental Health Endorsement.
