HS/PRS Case Manager - New England (RI + CT)

About the role

The HS/PRS Case Manager is responsible for managing cases in Rhode Island and Connecticut, ensuring timely and effective service delivery.

Responsibilities

• Manage caseloads in RI and CT, providing comprehensive support to clients.
• Collaborate with local community partners to enhance service delivery.
• Ensure compliance with all relevant regulations and policies.
• Conduct regular case reviews and provide necessary adjustments as needed.

Requirements

• Bachelor's degree in Social Work, Psychology, or related field.
• Minimum 2 years of experience in case management or related field.
• Strong communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.

Pay

Schedule

Full-time position, Monday through Friday, 8:00 AM - 4:30 PM.
