About the role

The HS/PRS Case Manager is responsible for managing high-risk patient cases, ensuring timely and accurate documentation, and coordinating care across multiple departments.

Responsibilities

• Manage complex patient cases with high risk factors
• Ensure timely and accurate medical record documentation
• Coordinate care among various healthcare providers and departments
• Provide clinical guidance and support to other team members
• Participate in interdisciplinary case conferences

Requirements

• Bachelor's degree in Healthcare Administration, Nursing, or related field
• Minimum 2 years of relevant healthcare experience
• Current certification in CPR
• Strong organizational and communication skills

Qualifications

• Experience with electronic health records (EHR)
• Knowledge of healthcare regulations and compliance
• Ability to work independently and as part of a team

Pay

$50,000 - $60,000 annually

Schedule

Monday through Friday, 8:00 AM - 4:30 PM
