About the role
The HS/PRS Case Manager is responsible for managing high-risk patient cases in collaboration with healthcare providers and insurance companies.
Responsibilities
- Assess and document patient needs and medical history
- Coordinate care plans with healthcare providers and insurance companies
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients and families to ensure understanding of their case
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in healthcare, social work, or related field
- At least 2 years of experience in healthcare management or case management
- Strong communication and interpersonal skills
- Proficiency in Microsoft Office Suite
Qualifications
- Knowledge of healthcare regulations and insurance policies
- Ability to manage multiple tasks and prioritize effectively
- Excellent organizational and time management skills
