About the role
The HRSN Navigator / Care Manager plays a critical role in helping Medicaid members access the supports they need to thrive in the community. This work will be completed within the Finger Lakes Region (Ontario, Wayne, Seneca and Yates Counties).
Responsibilities
- Use the AHC HRSN screening tool to identify needs.
- Connect individuals to community-based resources using the FindHelp platform.
- Provide follow-up and navigation support to ensure services are accessed.
- Build partnerships with libraries, DSS, and other community resources to increase awareness of the program.
- Track outreach and referrals, supporting Bridges and FLIPA in meeting program goals and demonstrating impact.
- Advocate for clients and provide ongoing encouragement to support independence and improved well-being.
Requirements
- Preferred Education & Experience:
- Bachelor’s degree in social work, psychology, sociology, or related field.
- At least two years’ experience providing direct services or connecting people to community resources.
- Strong knowledge of local community resources and willingness to build new partnerships.
- Excellent communication, organization, and problem-solving skills.
- Valid NYS driver’s license and reliable transportation.

Pay
Pay Range: $20–$23 per hour. Salary offers are commensurate with education and experience.
