About the role
The HR Manager at Oak Park Place oversees admission systems, controls, and alliances with healthcare networks while working closely with community liaisons, hospital discharge planners, and center staff to ensure smooth resident transitions into long-term care, memory care, and the rehab unit. This role also performs RN assessments for the Assisted Living side and may assist other campuses with their admissions processes.
Responsibilities
- Oversee admission systems, controls, and alliances with healthcare networks
- Work with Community Liaison and hospital discharge planners
- Serve as a point of contact to support colleagues and ensure the admission process runs smoothly
- Prepare paperwork, rooms, and staff for new admissions in long-term care, memory care, and the rehab unit
- Perform assessments for the Assisted Living side of the facility
- Work closely with the Licensed Nursing Home Administrator and center staff to ensure a smooth transition into the center
- Consult and coordinate with healthcare team members to assess, plan, implement, and evaluate resident care plans
- Perform RN assessment on all new admissions in a timely manner
- Manage the nursing admission process, including accurate completion of all required paperwork, completing all physician orders, proper notification, and proper documentation
- Assess, plan, and evaluate care delivery systems while monitoring resident outcomes
- Participate in resolution of resident, family, and staff compliments, complaints, and concerns
- Maintain “on call” status for emergencies on a rotating basis
- Complete and document Weekly Assessment tools
- Utilize established standards/criteria to oversee resident care services
- Potentially help other campuses with their admissions process and systems
Requirements
- Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle challenging situations
- Attentive to details
- Previous experience dealing with workers comp, unemployment, and employee relations
- Previous experience working with scheduling, preferably in a healthcare setting
- Previous experience dealing with employee relations
