About the role
This is a professional position providing Case Management services in the Family Works program under the Housing First Division. Family Works works with children and their families to decrease homelessness and achieve stable and permanent housing. The role involves assessing, linking, coordinating, and monitoring services from mental health, social, and educational aspects.
The Housing Stability Case Manager plays an integral role of support, linkage, coaching, and guidance for youth and families who are homeless or housing unstable and living with mental health or co-occurring mental health and substance abuse concerns, including trauma. Responsibilities include guiding youth and families in treatment/recovery by providing case management services to increase stability in the home.
The Housing Stability Case Manager functions as a member of an Integrated Decision Team (IDT) consisting of the Housing Stability Case Manager, a counselor, and an IDT facilitator. The role ensures the development of an individualized, strength-based treatment plan built collaboratively with the IDT team, family, natural supports, and formal supports. Services are delivered using trauma-informed care principles and are culturally and linguistically competent, tailored to the unique culture of the child and family.
This is a field position, with services provided in-home or in the community. The Housing Stability Case Manager is responsible for ensuring an annual client-facing care number of 1,410 hours. Achieving the required weekly, monthly, and annual client-facing hours is an essential function of this position.

Responsibilities
- Assess, link, coordinate, and monitor services from mental health, social, and educational aspects.
- Collaborate with families to develop service plans and identify needed services to establish and maintain stable housing.
- Provide case management interventions, support for clinical interventions, and utilize natural supports, community supports, and resources from other agencies.
- Function as a member of an Integrated Decision Team (IDT) to develop strength-based, individualized treatment plans.
- Report and discuss progress, barriers, interventions, safety issues, and other relevant information at IDT meetings.
- Ensure services are delivered using trauma-informed care principles and address how trauma impacts family functioning.
- Ensure services are culturally and linguistically competent, tailored to the unique culture of the child and family.
- Provide services in the community at locations convenient and comfortable for the youth and family.
- Maintain an annual client-facing care number of 1,410 hours.
Requirements
- Bachelor's Degree in Social Work or a related area of study from an accredited university; or a Master's Degree in Social Work or related area of study.
- Experience with child welfare, early childhood education, or children's mental health preferred.
- Knowledge of community resources.
- Knowledge of child development, including physical, mental, emotional, and cultural issues preferred.
- Computer literate and proficient in Outlook, Excel spreadsheets, and Microsoft Word; ability to work in a Windows environment and learn other software applications quickly.
- Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment (preference given to those already certified).
- Must become and maintain BHCM certification.

Pay
$32.44 per hour.
