Housing Specialist

About the Role

The Housing Specialist is a Bachelor’s level clinician who outreaches clients in the community to provide treatment and services with high fidelity to the ACT model. The Housing Specialist provides psychoeducation and support for clients’ housing needs, including submitting HRA2010e supportive housing applications, coordinating with shelter or current housing providers, assessing and promoting clients’ housing readiness, exploring clients’ skills and needs in obtaining and maintaining independent living, and advocating for clients to receive appropriate housing and supports.

The purpose of Assertive Community Treatment (ACT) is to deliver comprehensive and effective services to individuals (age 18 and up) diagnosed with serious mental illness (SMI) and whose needs have not been well met by traditional service delivery. These individuals may also have co-occurring substance use disorders and histories of criminal legal system involvement. ACT is an evidence-based practice providing integrated treatment, rehabilitation, case management, and support services through a mobile, multidisciplinary team to support recovery.

Responsibilities

• Provide at least 80% of treatment contacts in the community and complete expected minimum monthly treatment contacts, including leading groups when clinically indicated.
• Attend and participate in daily team meetings to discuss clients’ clinical and case management issues and needs.
• Complete documentation required by the NYS Office of Mental Health (OMH), NYC Department of Health and Mental Hygiene (DOHMH), NYS Medicaid billing guidelines, and CASES ACT Policy and Procedures.
• Collaborate with clients’ support networks and other service providers on treatment planning, service coordination, and case management to help clients obtain and maintain stable housing.
• Provide support for clients’ housing needs, including identifying risks, submitting housing applications (HRA2010e), preparing clients for housing interviews, and escorting clients to interviews.
• Provide clinical services that are anti-racist, gender-responsive, culturally humble, trauma-informed, recovery-oriented, client-centered, and evidence-based.
• Complete required documentation and toxicology tests for clients with active Assisted Outpatient Treatment (AOT) orders.
• Provide 24-hour crisis intervention on-call services on a rotating basis.
• Deliver culturally competent services in accordance with CASES policies and practices.

Requirements

• Bachelor’s degree.
• Experience working with individuals with serious mental illness and/or co-occurring substance use disorders.
• Experience with HRA2010e supportive housing applications and familiarity with housing resources for ACT-eligible populations.
• Highly organized, self-motivated, and a team player.
• Proficient in computers.

• Experience working with individuals involved in the criminal legal system.
• Fluency in Spanish.

Pay

The salary for this role is $61,800.

Schedule

Full-time, Monday – Friday, 9:00 AM – 5:00 PM (35 hours per week excluding breaks).

Flexible work arrangements allow staff to complete their workday from home or another suitable space, typically after reporting to the office and/or engaging in field or community-based work.
