About the role
OCES is growing! We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible. Our mission: Through the talents of an experienced and diverse workforce, OCES supports the independence and dignity of older adults and individuals with disabilities by providing essential information and services that promote healthy, safe living which positively impacts our community.
Responsibilities
- Review the needs of residents to link to community programs and services, improving quality of life and general well-being.
- Develop and maintain close working relationships with the LHA, site tenant association(s), OCES' Care Manager, and other community providers.
- Serve as the Multi-disciplinary Assessment Team Leader and meet with the team regularly to assure coordination of services for residents.
- Manage the provision of emergency and one-time only service(s) for residents as appropriate.
- Serve as liaison between LHA and OCES.
- Participate and/or facilitate care conferences with residents, family, and other participants to educate about service needs, provide and solicit advice about specific resident issues and resources, and resolve conflicts.
- Complete required reports for OCES, HA and other funding sources as needed.
- Develop and implement outreach/marketing plans.
- Plan and coordinate structured social, educational, and recreational programs.
- Prepare and distribute a periodic newsletter to keep residents informed of planned activities.
- Other duties as assigned.
Requirements

Qualifications
- At least two years of experience in the human services field, preferably with the elder population.
- Bachelor's degree in Social Work or related field preferred.
- Current Social Work License preferred.
- Bilingual a plus; Haitian Creole or Cape Verde Creole is preferred for one location.
