Housing Specialist

LaSante Health Center is seeking a compassionate and organized Housing Specialist to support patients experiencing housing instability, eviction risk, homelessness, rental arrears, and other housing-related challenges. Responsibilities: Assess patients’ housing needs and develop action plans.Assist with affordable housing, supportive housing, NYCHA, Section 8, shelter, rental-assistance, and eviction-prevention applications.Help patients gather documents and complete applications.Coordinate with landlords, housing agencies, legal providers, shelters, and community organizations.Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.Track applications, deadlines, and patient progress.Work closely with therapists, case managers, and medical providers. Qualifications: Experience in housing services, case management, social services, or a related field preferred.Familiarity with New York City housing resources required..Strong communication, organization, documentation, and follow-up skills.Ability to work compassionately with vulnerable individuals and families.High school diploma or equivalent requiredBilingual skills are a plus
