Housing Specialist

Duties And Responsibilities

• Aid FACT Team recipients in securing and maintaining appropriate housing.
• Show understanding of various services like Medicaid, Medicare, DSS, and SPOA housing.
• Make referrals and link clients to necessary community benefits, organizations, and institutions.
• Coordinate and provide services in the office or field to support case management and care coordination.
• Apply for and maintain benefits through the Department of Social Services, Social Security Administration, and housing applications.
• Facilitate the sharing of data with clients to promote partnered decision-making and keep them informed of progress.
• Participate in Quality Improvement (QI) training and attend refresher sessions.
• Attend agency-wide trainings and work groups, as well as NYS-specific trainings on the FACT model.
• Cultivate a culture of inclusion and respect among employees.
• Adhere to safety protocols and procedures to ensure a safe work environment.
• Complete QI training during onboarding and participate in refresher training as needed.

Qualifications

• Bachelor's Degree in social work, psychology, rehabilitation counseling, or a related field required.
• One year experience working in a mental health setting at the Masters level or two years' experience at the Bachelors level required.

The mission of the CN Guidance FACT Team is to deliver comprehensive and effective services to individuals diagnosed with severe mental illness, providing an integrated set of evidence-based treatment and support services through a highly individualized approach. Services are delivered in the community in settings where problems arise.
