CHD is seeking a Housing Specialist for our Adult Community Clinical Services (ACCS) in Springfield, MA. ACCS is a Department of Mental Health-funded program that provides a multidisciplinary outreach approach for individuals living independently in the community who need psychiatric and other services. This role involves active engagement, including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support.

The ACCS team includes licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists, working collaboratively to provide person-centered, evidence-based services in support of participants' treatment goals.

Responsibilities

  • Helps develop client treatment plans.
  • Initiates contact with participants and engages them in gathering housing history.
  • Develops a focused strategy for each person or family related to self-sufficiency, permanent housing, and strengthening economic stability.
  • Assists participants in finding appropriate housing.
  • Provides tenant education regarding rental and landlord responsibilities.
  • Maintains constant open communication with landlords, building trusting relationships to ensure longevity in permanent housing placements.
  • Provides outreach services to a caseload of participants.
  • Works within the agency’s team model, establishing open communication, support, and accountability among co-workers.

Requirements

  • High School Diploma or GED required; Bachelor’s degree in a related field (Psychology, Social Work, etc.) preferred.
  • 1 to 2 years of experience in a mental health-related treatment program.
  • MAP Certified preferred.
  • Valid Driver’s License and reliable vehicle for work purposes.

$21/hour.
