Housing Navigator - Bilingual Preferred (Russian/Farsi/Spanish)
About the role
Elica Health Centers is seeking a Housing Navigator to join our growing Community Supports (CS) program. The CS program provides free services to Medi-Cal beneficiaries with complex health issues, aiming to help them establish and maintain stable housing.
Responsibilities
- Develop and implement housing plans for clients experiencing homelessness, focusing on breaking the cycle of homelessness and maintaining housing stability.
- Facilitate clients into permanent housing, ensuring they have access to all necessary resources and services.
- Engage with community housing agencies and landlords to find suitable housing options for clients.
- Support clients with housing applications, deposits, and tenancy, coordinating with various benefit programs and services.
- Advocate for clients with benefits like Medi-Cal, CalFresh, SSI, EDD, IHHS, and WIC.
- Coordinate with landlords to ensure housing environments are safe and ready for move-in.
- Provide independent living and life skills training, including budgeting and connecting clients to community resources.
- Document encounters and services in Electronic Health Records (EPIC) and other systems.
- Manage caseloads of up to 40-45 active cases, attending regular meetings and community outreach events.
Requirements
- High School Diploma and one year of housing navigation or case management experience.
- Associates degree or lived experience preferred.
- Bachelor’s Degree in Social Services or related field preferred.
- CPR and first aid certificate or willingness to be trained within 90 days.
- Experience working with homeless and vulnerable populations.
- Experience with Electronic Health Record System (EPIC).
- Experience using HMIS training highly desirable.
- Commitment to HIPAA and ability to maintain confidentiality.
- Excellent organizational and time management skills.
- Strong interpersonal, persuasion, and problem-solving skills.
- Ability to work under pressure and meet established goals.
- Ability to work Monday through Friday, 8:00 AM to 4:30 PM.

- Current California driver’s license and proof of personal auto insurance.
- Proof of immunizations, tuberculosis clearance, and COVID-19 vaccination status.
- Passing criminal background investigation and drug screen.
