Housing Case Manager - The 101 Apartments

We’re looking for dedicated team members to join one of California’s largest nonprofit developers of high-quality affordable housing and supportive services that transform lives and strengthen communities. This property is part of an Intensive Services Care Program in collaboration with the County of Los Angeles. Rooted in a Housing First model and guided by a trauma-informed care approach, this position supports residents through case management services in collaboration with the County.

About the Role

The Housing Case Manager will implement and coordinate programs that develop residents’ capacity to engage in life skills development, coping mechanisms that nurture their well-being, set personal goals, and maintain housing stability. The role also provides linkage and referral services to residents to increase housing stability.

Responsibilities

• Provide supportive services to address issues and barriers that negatively impact progress toward goals.
• Maintain tenant rosters showing entry-exits for the property.
• Monitor for warning signs of de-compensation and crisis, and respond accordingly.
• Handle record keeping, documenting, and reporting as needed.
• Document resident participation in services and track progress of resident goals.
• Complete monthly newsletters describing available social and supportive services on-site and off-site, including a calendar of services.
• Maintain information on off-site services and programs and provide referrals when appropriate.
• Provide advocacy, onsite case management services, and linkage to residents as needed.
• Complete intake assessment and orientation with new residents.
• Maintain effective relations and coordinate services between community supports, service providers, and property management.
• Foster a positive atmosphere for residents and promote community engagement.
• Facilitate daily Adult Education groups/workshops onsite for residents.
• Plan and coordinate Health and Wellness events and community-building activities yearly.
• Input data into the Homeless Management Information System (HMIS) accurately and on a timely basis.
• Provide onsite life skills training and reinforcement to residents.
• Other duties as assigned.

Requirements

• Over 21 years of age.
• Experience working with individuals experiencing chronic homelessness and mental health challenges.
• Familiarity with Housing First, Harm Reduction, and/or Recovery-Oriented service models.
• Ability to maintain relationships with consumers/members while maintaining appropriate boundaries as staff.
• Ability to interact in a culturally diverse community.
• AA degree preferred; or equivalent experience serving homeless individuals.
• Ability to work effectively in an interdisciplinary team setting and independently.
• Knowledge of independent living skills such as cooking, budgeting, housekeeping, and being a good neighbor.
• Valid California driver’s license and availability of an insured vehicle.
• Job includes some evening and weekend hours and travel throughout California.

Pay

$24–28/hour + benefits.

• Required to talk and hear.

• Required to stand, walk, reach with arms and hands, climb, balance, twist, lean, move between locations, stoop, kneel, crouch, or crawl.
• Vision abilities include close and far vision.
• Ability to operate a computer keyboard and lift files and reports.
• Exposure to glare from a computer.
• Must be able to lift 25 lbs and travel by automobile 10% of the time.

Work Environment

• Working with the homeless population in various settings, including streets, shelters, or other places of habitation or services.
• Exposure to bed bugs or other infestations, unpleasant smells or odors, and unclean environments due to homelessness, mental health symptoms, or poverty.
• Some work will occur in a recreational room or common area.
• The noise level is usually moderate but varies depending on the activity.
