Catholic Charities seeks a dedicated Case Navigator to join our compassionate team in the Houston-Galveston area. We are committed to serving our community through a family-friendly organization that invests in its employees.
Responsibilities
- Coordinate and update referral services
- Provide input on complex cases
- Maintain client files according to agency policies
- Input client information into a designated database
- Gather and report program metrics
- Assess families and individuals for strengths and barriers, including employment, behavioral health, criminal history, and substance use
- Refer households to appropriate levels of housing and rental assistance
- Absorb caseloads of families and individuals, including home-based case management
- Document case management services precisely and accurately, including client files and entries into the program databases
- Coordinate with various outside providers to meet client needs and avoid duplication of services
- Provide community resource referrals for clients
- Perform other duties as assigned
Qualifications
- Bachelor degree in social work, psychology, or related field required
- One year of experience in working with homeless populations, including individuals with mental health challenges
- One year of experience in case management, supportive housing, or homelessness services
- Valid Texas Driver's License
Skills and Assets
- Experience conducting outreach and presentations
- Flexible and effective team player, adaptable to changes and busy workplace environments
- Knowledge and experience with housing issues and available resources
- Excellent organizational skills and self-motivation
- Excellent written/verbal communication and interpersonal skills
- Computer literate - experience with MS Office Suite (Word, Excel, Outlook) and internet
- Ability to work under pressure, manage multiple priorities, and meet deadlines
- Ability to identify key issues in problem-solving and implement effective and creative solutions
Pay
Negotiable based on experience and qualifications.
