House Call Physicians/Home Based Primary Care

About the role

VPA is seeking physicians to join its team in the Jacksonville area. This opportunity offers competitive base salary and aggressive quarterly incentive pay, with an income potential of up to $170,000.

We provide a comprehensive benefit package including a 401K with a match, an annual stipend for CME and license/association dues, and UpToDate on tablets and phones. We also offer A+ rated malpractice coverage with tail coverage, no nights, no holidays, and minimal call responsibilities.

Responsibilities

• Provide quality medicine and compassionate care to patients who cannot access necessary care in a traditional practice setting.
• Participate in clinically integrated network (CIN), population health management, and value-based healthcare initiatives.
• Implement best practice protocols including COPD, Pneumonia, CHF, Diabetes, and Chronic Care Management.
• Utilize a web-based EHR for all clinicians and follow fully implemented compliance, QAPI, infection prevention and control plans.

Requirements

• Board certified or board eligible in Family Medicine, Internal Medicine, or Pediatrics.
• Experience in home-based primary care preferred.
• Strong communication skills and ability to build rapport with patients and families.

Qualifications

• Must be licensed to practice medicine in the state of Florida.
• Pass a background check.
