About the role
The Contract Monitor oversees specified Host Homes in the Eastern Iowa service area, responsible for the oversight of services and communicating with interdisciplinary team members. Contract Monitors ensure contractual compliance and that assigned independent providers deliver quality care while meeting contractual requirements. This position is centralized in the Cedar Rapids, Cedar Valley, Davenport, or Dubuque area and includes some work-from-home flexibility along with travel within the following counties: Allamakee, Benton, Black Hawk, Bremer, Buchanan, Butler, Cedar, Chickasaw, Clayton, Clinton, Delaware, Dubuque, Fayette, Floyd, Howard, Jackson, Johnson, Jones, Linn, Louisa, Muscatine, Scott, and Winneshiek. Travel outside these counties may occasionally be required to cover service needs and meetings.
Responsibilities
- Act as the main contact for interdisciplinary team members and individuals served in the host home program.
- Work directly with assigned host homes to provide support, assistance, and instruction in areas that facilitate personal growth and independence.
- Facilitate and participate in interviews and evaluations related to the contracting process for independent providers.
Schedule
This is a salaried position that generally follows standard Monday–Friday business hours. Flexibility may occasionally be required to work outside regular hours, including some evenings or weekends, depending on business needs. The role also includes participation in an on-call rotation.
Requirements
- Minimum Qualifications:
- HS Diploma/GED and five years of related experience, OR
- Associate’s Degree and three years of related experience, OR
- Bachelor’s Degree and one year of related experience.
- Preferred Qualifications:
- Bachelor’s Degree in a human services-related field and one year of related experience.
- Years of related experience in a supervisory or leadership role.
- Valid driver’s license, auto insurance, and a good driving record.
- Ability to pass an extensive background check, including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry, and a criminal history check.
