About the role

Concierge Home Care is hiring a Hospital Liaison (RN or LPN) to serve as the connection between hospital teams, physicians, patients, and home health services. This role bridges the gaps in transitional care and focuses on building referral relationships, supporting smooth hospital-to-home transitions, educating patients and families, coordinating post-discharge care, and driving admissions growth within the assigned hospital systems.

Coverage Area: Orange Park Medical Center and Surrounding Areas.
