Responsibilities
- Participate and lead pharmacy clinical/operational programs
- Monitor patient medication regimens and provide appropriate interventions
- Review, interpret, and input treatment orders to ensure accuracy
- Support clinical pharmacy responsibilities by collaborating with other hospital disciplines
- Lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs
- Provide medication and practice-related education and counseling to patients, caregivers, and medical staff
- Collaborate with medical and nursing staff to optimize pharmacotherapeutic aspects of patient care
- Provide information to staff members and/or patients on medication uses, drug interactions, and potential side effects
- Oversee the preparation of treatments, preparation of investigational treatments, prepackaging of medications, and drug returns
- Maintain records on patient prescriptions, charges, and inventory
- Ensure pharmacy technician duties are performed completely, safely, and without risk of harm to patients
- Analyze, compare, and evaluate various courses of action; make independent decisions
- Manage stem cell transplant and cellular therapy therapies, order sets, and REMS requirements
- Provide staff education regarding cellular therapy management
- Provide direct clinical pharmacy support for stem cell transplant and cellular therapy patients
- Aid in the precepting of pharmacy students and residents
- Perform all other duties as assigned
Qualifications
- Knowledge of and compliance with state and federal pharmacy laws and regulations
- Excellent communication skills
- Organization and problem solving skills with the ability to work in situations involving uncertainty and rapid change
- Ability to align all patient care activities and project work with departmental and institutional policies, business model/needs, goals, and vision
- Thorough understanding of dosage administration and measurement, chemical compounds, medical brands, etc.
- Minimum of 3 years’ experience as Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience required; preferably within area of focus
