Hospital Case Manager

Job Summary

The Case Manager at Cleveland Clinic Inpatient Rehabilitation Hospital is responsible for coordinating health care decisions, promoting cost-effective care, and implementing safe and appropriate discharge plans. This position requires assessing psychosocial needs and providing intervention as part of the discharge planning process.

Responsibilities

• Payer/Referral Management

Requirements

• Current Licensure per state guidelines in clinical or related discipline
• Current licensure as a Case Manager preferred
• Bachelor's or Master's degree in a health or human services discipline
• Previous experience in Case Management and Discharge Planning preferred
• CCM Certification preferred

• Committed to providing exceptional patient care
• Part-time/Per Diem positions eligible for 401k after 1,000 hours within the first anniversary or subsequent calendar year
