Hospital Care Coordinator-RN Case Management

  • Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
  • Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
  • Captures and documents all relevant information about the patient's condition and treatment plan.
  • Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
  • Ability to adjust to changing priorities in a fast-paced environment.

Responsibilities

  • Provides linkage, monitoring, planning, and advocacy for our population to achieve and maintain maximum functioning.
  • Captures and documents all relevant information about the patient's condition and treatment plan.
  • Coordinates discharge planning efforts through assessment, referral, and interdisciplinary planning.
  • Ability to adjust to changing priorities in a fast-paced environment.

Requirements

  • RN (Registered Nurse)
  • Preferred ACM (Case Management)
  • Preferred CCM (Case Manager)
  • Education: Associates of Nursing or Bachelor of Nursing
  • Experience: 2 years in Acute Care

Qualifications

  • Current Florida RN license
  • ACCM certification preferred
  • CCM certification preferred

Skills

  • Excellent communication skills
  • Strong organizational and problem-solving skills
  • Ability to work independently and as part of a team

Part time, 16 hours per week
