Hospital Care Coordinator-RN Case Management
- Possesses a clear understanding of the available resources to promote quality cost effective outcomes.
- Effectively maintains and manages assigned caseload, contingent on needs, strengths, abilities, barriers, and preferences of the individual served.
- Captivates discharge planning efforts through assessment, referral, and interdisciplinary planning.
- Ability to adjust to changing priorities in a fast-paced environment.

- RN (Registered Nurse)
- Preferred ACM (Case Management)
- Preferred CCM (Case Manager)
- Education: Associates of Nursing or Bachelor of Nursing
- Experience: 2 years in Acute Care
Qualifications
- Location: Wesley Chapel Hospital
- Status: Part time, 16 hours per week
- Schedule: Saturday & Sunday - 8:00 AM - 4:30 PM
- Weekend Requirement: Every
- On Call: No
