About the role

As a Hospital Based Patient Advocate, you play a vital role in guiding uninsured hospital patients through the complex landscape of medical and disability assistance. This onsite, hospital-based role places you at the heart of patient financial advocacy—meeting individuals face-to-face, right in their hospital rooms, to guide them through the process of identifying eligibility and applying for financial assistance. Your presence and empathy make a real difference during some of life’s most vulnerable moments.

The purpose of this position is to connect uninsured hospital patients to programs that will cover their medical expenses. You will evaluate eligibility for federal, state, and county medical or disability assistance programs through bedside visits and in-person interactions, guiding patients through the application process to ensure thorough completion and follow-up. The goal is to meet our benchmark that 98% of patients are screened at bedside.

Responsibilities

  • Screen uninsured hospital patients at bedside to determine if they are viable candidates for federal, state, and/or county medical or disability assistance.
  • Complete the appropriate applications and follow through until approved.
  • Provide detailed, accurate, and timely documentation in both Elevate PFS and hospital systems for all cases worked.
  • Maintain exceptional customer service skills at all times.
  • Maintain an assigned work queue of patient accounts.
  • Collaborate in person and through verbal/written correspondence with hospital staff, case managers, social workers, and financial counselors.
  • Answer incoming telephone calls, make outbound calls, and track all paperwork necessary to submit enrollment and renewal for prospective Medicaid patients.
  • Maintain structured and timely contact with the applicant and responsible government agency, primarily by phone, or as structured via the daily work queue.
  • Assist the applicant with gathering additional reports or records, meeting appointment dates and times, and arranging transportation if warranted.
  • Conduct in-person community visits as needed to acquire documentation.
  • Inform the client in a timely manner of all approvals and denials of coverage, as per established protocols.
  • Attend ongoing required training to remain informed about current rules and regulations related to governmental programs.
  • Regular and timely attendance.
  • Other duties as assigned.

Requirements

  • Some college coursework preferred.
  • Prior hospital experience preferred.
  • Adaptability when dealing with constantly changing processes, computer systems, and government programs.
  • Professional experience working with state and federal programs.
  • Critical thinking skills.
  • Ability to maneuver throughout the hospital and patients’ rooms throughout the scheduled work shift.
  • Proficient experience utilizing Microsoft Office Suite with emphasis on Excel and Outlook.
  • Effectively communicate both orally and in writing to a variety of individuals.
  • Ability to multitask to meet performance metrics while functioning in a fast-paced environment.
  • Dress in accordance with the hospital’s Dress Code.
  • For hybrid positions: home internet connection meeting the Company’s upload and download speed criteria, and compliance with Elevate’s Remote Work Policy, including working in a private and dedicated workspace where confidential information can be shared in accordance with HIPAA and PHI requirements.
