Job Description

The Hospital Based Admission RN plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. Provides appropriate palliative and hospice care through assessment, teaching, counseling and direct care to client and/or family members. Coordinates care with hospital providers and clinical professionals. Coordinates care with other Hospice Interdisciplinary Team (IDT) team members and attends IDT meetings as indicated.

Essential Duties & Responsibilities

  • Describes and explains hospice services to a potential patient/family and elicits responses to questions regarding patient/family attitude toward hospice care, physical and psycho/social/emotional status, in order to put the patient and family at ease about choosing hospice and fully inform them about the program.
  • Gathers pertinent information necessary for admission. Speaks to patient/family about insurance coverage and financial obligations, using knowledge of hospice fees, cost and reimbursement sources.
  • Promotes hospice services, to ensure appropriate GIP referrals to hospice program to discharge planners, physicians, other health care providers and referral sources.
  • Manages care of patients, coordinate all services and facilitate communication between all providers
  • Affords assessments patient and coordinate cares with hospital care team including MD, Case Manager, RN and hospice team.
  • Provides expert symptom management and recommendations in coordination with the Hospice Medical Director.
  • Plans for continuity of care with nursing team members, patient, family, and other disciplines.
  • Responsible for daily nursing visits and assessment of ongoing eligibility of patients admitted to GIP level of care. Coordinates with the hospice IDT if the patient becomes no longer eligible and an alternative plan becomes necessary.

Licensure and Certification

  • Registered Nurse from a Graduate of National League for Nursing accredited school of nursing
  • BSN strongly preferred
  • Current license to practice as a registered professional nurse in the Commonwealth of Massachusetts
  • Currrent Basic Life Support Certification
  • Valid Massachusetts driver’s license

Experience

  • Minimum 3 years Hospice Experience
  • Inpatient hospice experience preferred
  • Hospice or end of life care experience required

Skills, Knowledge & Abilities

  • Must be sensitive to the needs of terminally ill patients and families and one's own feelings about dying and death.
  • Must demonstrate the ability to interact in a positive and courteous manner with patients, families, other employees, hospital staff and outside vendors.
  • The ability to work both independently and as part of a team are essential. Demonstrates excellent observation and problem solving/critical thinking skills. Well-developed verbal and written communication skills; essential computer skills to work with EMR.
  • Self-directed and able to work with minimal supervision
  • Shows strong ability to organize and prioritize workload independently; nursing skills per competency checklist.

Pay Range

$69,600.00 USD – $103,120.00 USD
